Project Overview and Indicators

AFMC’s Asthma Project works to improve care for asthma patients across the state. The project focuses on three main goals to decrease asthma emergency room use and hospitalizations:

• Increase follow-up visits for patients previously hospitalized with asthma complications
• Help patients and their families recognize the potential for a normal life with appropriate asthma therapy
• Increase physician recognition of chronic asthma and treatment of the condition with anti-inflammatory medications

Indicators

• Anti-inflammatory medications for mild persistent to severe asthma
• Regular use of objective peak flow monitoring
• Asthma Action Plan written and placed on each asthmatic chart with copy given to patient and caregiver
